Work Request Form
Please fill out the form below if you are ready to have samples analyzed. After the information has been received, an official document will be mailed to you along with a guide to the facility and a handful of sample submission request forms to get you started. The Technical Testing Agreement is a legal document designed to protect your research interest by keeping that information proprietary and also alerting you, the user, to the operational policies of the facility.
Once you receive the document, simply sign at the marked spots and return to the lab. It's that simple!
